From Travel Weekly: "SANDALS AND BEACHES RESORTS went smoke-free May 1 in all enclosed areas, including nightclubs, bars, restaurants, lobbies, conference facilities and retail shops. Smoking is still permitted outdoors and in designated rooms in each of the resorts. For details, visit www.sandals.com and www.beaches.com.

Sandals Smoking Policy

Yes they do have an advertised non-smoking policy, but in reality it is not implemented.
Restaurants that are enclosed by 4 walls are non-smoking, and as a non-smoker you are safe here. If the restaurant is not enclosed by all 4 walls - forget it !!!

Anyone can and they do light up in lobbies, rooms, bars (not all though) entertainment areas (not all) etc etc. Sandals does not advertise the truth.

Sandals has fallen short of their own standards.

Staff are obviously told not to actively enforce it.
